Diarrhoea of Acute Kwashiorkor

چکیده
Children with kwashiorkor almost invariably have diarrhoea which may be severe and prolonged. The exact cause is often obscure and the management unsatisfactory (Brock et al., 1955; Trowell, 1958; Pretorius and Smit, 1958). Recently Bowie, Brinkman, and Hansen (1965), and Bowie, Barbezat, and Hansen (1967) found that the majority of their kwashiorkor patients with severe diarrhoea responded with a dramatic reduction in daily stool weights on a lactose-free diet, with or without added glucose. The diarrhoea was related to lactose malabsorption as a result of intestinal lactase deficiency. Other disaccharidases were much less affected, and absorption tests for sucrose and maltose were normal. In Uganda, Wharton (1968) found intolerance to disaccharides and glucose in kwashiorkor patients, and he was able to control the diarrhoea with a diet containing fructose as the only carbohydrate. The present investigation was undertaken to evaluate further the effect of different sugars on the diarrhoea of kwashiorkor in an area where life-threatening diarrhoea is less frequently encountered than reported from other centres (Pretorius, Wehmeyer, and Mey, 1964; Wharton, 1968).

Oedema in kwashiorkor is caused by hypoalbuminaemia
It has been argued that the oedema of kwashiorkor is not caused by hypoalbuminaemia because the oedema disappears with dietary treatment before the plasma albumin concentration rises. Reanalysis of this evidence and a review of the literature demonstrates that this was a mistaken conclusion and that the oedema is linked to hypoalbuminaemia. This misconception has influenced the recommendations ...
متن کاملDiarrhoea in kwashiorkor.
Diarrhoea was a common problem in the kwashiorkor seen in Kampala, contributing to the mortality and delay in recovery. Enteric infection was found in only a few children (8%), but when present it caused particularly severe diarrhoea and was frequently complicated by septicaemia.Sugar intolerance often occurred to lactose and other sugars, both monosaccharide and disaccharide. The children were...

The pre-dietary rehabilitation levels of acute phase proteins (APP) namely, alpha-1-antitrypsin (AAT), orosomucoid (ORO), haptoglobin (HAP), fibrinogen (FIB) and C-reactive protein (CRP) in the plasma of Sudanese children with severe protein energy malnutrition (PEM) were compared with those of normal controls, and with the levels after dietary rehabilitation. Eighty one children were included ...
متن کاملProtein and fat absorption in prolonged diarrhoea in infancy.
15N-yeast protein absorption, nitrogen and fat retention, stool reducing substances, and lactate concentrations were measured in 22 infants who had had severe diarrhoea for 7 days. Stool losses of nitrogen and fat were large, and an appreciable proportion appeared to be endogenous. The supply of nitrogen and energy might have been a limiting factor in tissue repair if stool weight exceeded 30 g...
متن کاملEffect of lactose-induced diarrhoea on absorption of nitrogen and fat.
The absorption of fat and the absorption and retention of nitrogen were studied in 20 children with kwashiorkor. 13 of the 20 had lactose malabsorption and severe diarrhoea on milk feeds. Fat absorption was not affected by lactose-induced diarrhoea, but nitrogen absorption was impaired. Despite this, nitrogen retention was unaffected and it is concluded that milk and milk products would continu...
